These Terms and Conditions apply to all patients receiving services from Total Health Physiotherapy Ltd. By booking and attending an appointment, you agree to the following terms. Appointments and Booking Appointments can be booked via the online booking system (Rehab Guru), or by direct communication. It is your responsibility to ensure that all information provided at the time of booking is accurate and up to date. Fees and Payment All fees are payable at the time of your appointment unless otherwise agreed in advance. Total Health Physiotherapy Ltd reserves the right to amend fees at any time. Patients will be informed of any changes prior to their appointment. Cancellation Policy A minimum of 24 hours’ notice is required to cancel or reschedule an appointment. Failure to provide adequate notice may result in the full appointment fee being charged. Late arrivals may result in a shortened session, and the full fee will still apply. Clinical Responsibility All treatment is provided based on clinical assessment and professional judgement. While every effort is made to achieve the best possible outcome, no guarantees can be made regarding treatment results. Patients are responsible for: • Providing accurate and complete medical information • Informing the practitioner of any changes to their health • Following advice and treatment recommendations where appropriate Consent to Treatment By attending your appointment, you consent to assessment and treatment. • Written consent is obtained at your initial appointment • Verbal consent is confirmed at each session • Additional explicit consent is obtained for any intimate examination You have the right to withdraw consent at any time. Chaperones A chaperone will always be offered for intimate examinations. As a sole practitioner, Total Health Physiotherapy Ltd is unable to provide a chaperone. Patients are welcome to bring their own chaperone if they wish. Your decision regarding a chaperone will be documented. Children and Young People Patients under the age of 18 must be accompanied by a parent or responsible adult at all times. Consent must be provided by a parent or legal guardian. Confidentiality and Data Protection Your personal data will be handled in accordance with our Privacy Policy and UK GDPR. Information shared during treatment is confidential except where disclosure is required: • By law • For safeguarding purposes • To prevent serious harm Complaints If you are dissatisfied with any aspect of your care, you are encouraged to raise your concerns directly. A formal complaints procedure is available and can be provided upon request. Liability Total Health Physiotherapy Ltd holds appropriate professional indemnity insurance. Nothing in these terms excludes liability where it would be unlawful to do so. Changes to Terms These Terms and Conditions may be updated from time to time.
